[collectd] [Patch] Added SeparatePluginInstances and SeparateTypeInstances options to write_graphite plugin

Hi!!

I'm working on improve  the format tree you can see in Graphite, so I have
separated  the "SeparateInstaces" Option in two diferent Options:
SeparetePluginInstances and SeparateTypeIntances.

 I have kept The "SeparateInstances" option for backwards compatibility.
